The purpose of the Road
to Excellence initiative is to:
n Demonstrate in measurable ways the
proactive commitment of the long term care profession to overall care
improvement
n Provide individual facilities with
the tools and resources necessary to positively impact resident care in targeted areas
n Monitor our success and provide data
driven evidence of the care improvements being made as a result of increased
funding for the care of nursing home residents.
Facilities participating
in the program will:
n Focus clinical efforts on reducing pressure sores and managing the pain of residents
n Link to national initiatives by
participating in the Illinois Foundation for Quality Health Care’s (IFQHC) free
on-line benchmarking STAR (Setting Targets, Achieving Results) program in the
two areas of pressure ulcers and pain
n
Adopt and implement a formal program to assess resident
and family satisfaction
n
Report program results quarterly for statewide
database development
The following resources
will be available to facilities participating in the program:
n Clinical protocols, assessment tools, reporting forms and documentation guides for pressures ulcers and pain management
n
Confidence Satisfaction Survey Program to conduct resident and family satisfaction surveys
n
Training seminars on pressure sore prevention and pain
management
n
Training seminars on conducting and using satisfaction
surveys
n
Satisfaction data and clinical benchmarking with
targets for improvement
n
Regular newsletters featuring best practices and resident
success stories
The Road
to Excellence initiative is intended to complement and support
existing facility efforts in preventing pressures ulcers, managing pain, and
assessing customer satisfaction. The
program is consistent with national programs but is specifically tailored to
